Discuss the phases of business cycle. Suggest Suitable Fiscal policy and monetary policy to overcome the recession in economy. Business Cycle – A business cycle is also known as trade cycle. It implies wave like fluctuations in the level of economic activity, particularly in national income a, employment and output. It is a short term picture of the behaviour of real output in a private enterprise economy. Business cycle refers to upturn and downturn in the level of economic activity that extends over a period of time. The business fluctuations occur in aggregate variable such as national income, employment and price level. The variables nearly move at the same time and in the same direction. However they vary in duration and intensity. Cyclic fluctuations have the following features: †¢Wave line movements – ocyclical fluctuations are wave like movements and are recurrent in nature ocharacterized by alternation of expansions( prosperity) and contraction ( depression) in economic activity oare repetitive and rhythmic ocontains oscillating movements in the form of waves from peak to trough and trough to peak †¢Synchronic – oEntire business of an economy acts like an organism oAny happening on economic front affects the entire economy oAnd through the mechanism of international trade affects entire world oexample -The Great depression 1929 †¢Cumulative – oProcess of expansion and contraction is of cumulative and self-reinforcing in nature oMoves in same direction until external forces reverses its direction †¢Self-generating forces – oIt can terminate the period of prosperity and start depression oCannot have definite period of prosperity or depression †¢Non identical – oPeriod of trade cycle are not identical although they recur with great regularity †¢Not symmetrical oPeak and trough are not symmetrical oMovement from upward to downward is more sudden and violent than that from downward to upward oDownturn is sharp and steep oIt is relatively narrow at its peak and flatter at its trough Phases of business cycle: A business cycle can be divided into four phases. They are shown in the fig 1.1 The phases are: 1.Expansion or prosperity or the upswing 2.Recession or upper-turning point 3.Contraction or depression or downswing 4.Revival or recovery or lower turning point These phases are recurrent and uniform in case of different cycles. But no phase has definite periodicity or time intervals. A business cycle starts from trough or low point, passes trough a recovery and prosperity phase, rises to peak, declines through a recession and depression phases and again reaches a trough. 1)Expansion or prosperity or the upswing or peak or Boom – The top of a business cycle is called Peak or Boom or prosperity phase. In the boom period, the overall business activity is rising at a more rapid rate. There is a rise in real output and incomes of the people. There is a rise in production, prices, employment, wages, interest rates, profits and in the volume of bank credit. The general mood of the businessmen is that of optimism and commercial. The industrial activity both speculative and non-speculative shows remarkable expansion. Construction activity gets a big boost. Share markets give good gains to the investors. Financial institutions tend to expand credit. In the words of Haberler. ‘‘Prosperity is ‘a state of affairs in which the real income consumed, real income produced and the level of employment are high or rising and there are no idle resources or unemployed workers or very few of either†. During prosperity period, there is a high level of demand for capital goods and consumer goods and services. Risky investments are undertaken by the entrepreneurs. Inefficient firms enter into the market and manage to survive. The high level of demand for the various inputs creates shortage of some of them. Inflation goes up. The economy becomes supply constrained. The state of prosperity proves to be short lived and the downturn of its period starts. 2) Recession or upper-turning point – The end of prosperity comes and enters into recession. Recession is a slowdown of business activity. In recession employment and output both decline. The forces which bring the contractionary phase of business fluctuations (recession) are as follows: a) As prices of the commodities rise the wages of the workers tend to lag behind. The reduction in the purchasing powers of the workers brings down the demand for consumer goods b) Due to shortage of some inputs the expansion in production of goods is hampered. c) The non-availability of credit beyond a particular limit of expansion acts as a serious brake on prosperity. The financial institutions begin to recover the loans. The firms which are unable to pay back the loans begin to liquidate their stocks. When more firms sell their output at the same time the price level starts falling. If a few firms get involved in losses a wave of pessimism runs through the share markets. The firms begin to curtail production. Workers are laid off. The outstanding orders for raw materials are cancelled. The new projects are shelved. The wave of pessimism passes on to other sectors of the economy and the businessmen become panicky and the whole economic system runs into crisis. Then the next stage of business cycle called depression starts. 3)Depression or Trough or downswing – Depression is the most fearful stage of a trade cycle. The phase of depression (also called slump) is characterized by low economic activities, rapid decline in general output and employment. The decline in economic activity is not uniform. There is much more decline in output in manufacturing mining construction transport industries. However there is comparatively less contraction in output in retail trade and agriculture. In slump, there is a marked fall in the average prices of the commodities. The costs are relatively higher the profits of the entrepreneurs decline. The purchasing power of the money is high but due to low income there is too much contraction in effective demand for consumer goods. The expenditure on capital goods or its replacement greatly falls Most of the firms reduce their output or close down. The income of the shareholders goes down. Depression or slump leads to redistribution of national income Profits and wages fall faster relatively to rent and other fixed incomes. The bankers  follow the policy of credit contraction. Due to dull business conditions producers are also reluctant to borrow funds Summing up in a period of slump there is negative net investment by firms falling demand of consumer as well as capital goods high unemployment and low level of imports. In the words of Haberler, Depression is a state of affairs in which real income consumed or volume of production per head and the rate of employment are falling. There are idle resources and unused capacity especially unused labour.’’ In the economic life of the world such acute crises have occurred in the years 1710 1827 1873 1907 and 1929. 4) Revival or Recovery – The economic conditions which we have described in depression phase do not remain as such forever. After sometime revival or recovery sets in under the influence of a variety of factors. The revival phase develops when the accumulated stock of commodities with the businessmen are exhausted. The costs under the impact of prolonged depression begin to fall. The prices which have reached its lowest level stop falling further. There is then complete harmony between costs and price relationship. When profits begin to reappear, the businessmen are induced to invest their hoarded money in some enterprises: In order to steal a march over other industrialists, they start repairs, renewal and replacements of their capital equipments and stocks. The capital goods industries resume activities. There is gradual of labor. The money incomes begin to increase and the effective demand is revived. The government also tries to break the spell of depression by starting construction or expanding some public works with a view to give more employment. The commercial, banks which have accumulated large reserve offer credit on favorable terms. The marginal efficiency of capital begins to rise and investment opportunities brighten up. The consumers start buying commodities to avoid the rise. Due to increase in demand for commodities, investment in various industries is stimulated and thus the revival takes place. The recovery phase of business cycle thus is characterized by rising production, increasing prices of both consumption and capital goods, rising of wages, rates, enlarged opportunities of employment, and greater amount of  spending on consumption and investment goods. Prior to 1940’s, there were frequent booms and depressions in the capitalistic world. However, after the World War-Il, the strong cyclical upswings and downswings have been considerably tamed by the timely applications of fiscal and monetary measures. The fluctuations in economic activity are now moderate. Consequently, the term economic expansion and economic contraction are used now for the terms boom and recession. Both expansionary monetary policy and expansionary fiscal policy are being used to counter the recession. Expansionary monetary policy is basically just lending more money to people; people borrow that money and spend it creating demand in the process. The United States has been using expansionary monetary policy for about 20 years straight now which has directly lead to massive increases in the levels of debt in the economy. Debt levels are so high now that no one can actually borrow any more so monetary policy has stopped working. Monetary policy is at the most expansionary setting possible right now and it is having effectively no expansionary impact on the economy as a consequence of excessive debt levels. Fiscal policy thus is the only option left available to actually rectify the situation; basically all you are doing is spending money through the government thus creating demand in the economy. Measures being taken include tax cuts, other measures being taken include infrastructure spending and extending the length of unemployment benefits. This is basically a Keynesian approach. Keynesian economics revolves around the concept of ‘aggregate demand’ the government can increase the amount of aggregate demand through government spending. A Keynesian approach is fundamentally the right way to go under the circumstances that exist as this crisis is basically a crisis of demand. The classical approach is to do nothing and to rely on the natural robustness of the macro economy to solve the problem. Most macroeconomists agree that this is ineffective if not stupid, but it was first thought to be the solution to the 1929 market crash. The Keynesian approach takes several forms but all of them are supposed to  result in the so called â€Å"multiplier effect† causing the economy to grow once it has been stimulated by making more money available at some place in the social system. Unfortunately it doesn’t work due to this money having to be borrowed or taken from some other part of the system. Keynesian economics is only a partial model and is unable to really show how it might grow. The current Keynesian methods in use are to borrow money from the public and increase the national debt. Also to print more money and use it to reduce this debt, but this means inflation and it is no more effective than that of the greater loans. Inflation is also dishonest because it makes the debt owed by the government of smaller value in terms of what its money can buy. Money is only a representative of wealth, not wealth itself. If the system were one of barter and in the present crisis then more money does not mean more wealth, except for the printers of course. To reduce the rate of interest on the national debt does help to reduce the budget deficit for the next year, but it is not very effective and will not solve the problem at anything like the speed needed. So that with present methods there is no way to get out of the recession. Fiscal Policy When it comes to how fiscal policy affects the economy during a recession, the government has some automatic stabilizers in effect. These items work to automatically stabilize the economy when a recession takes place. With fiscal policies, the government influences the economy by changing how it (the government) spends and collects money. For example, the income tax system acts an automatic stabilizer. When people make less money, they also pay less money in income taxes. Unemployment benefits are another example of an automatic stabilizer. This helps families continue to receive income so that they can keep spending and keeps the economy going. The most common fiscal policy actions in a recession are: †¢Tax cuts for businesses or for individuals – When the economy is struggling during a recession, the government can attempt to help the situation by charging less in taxes. In many cases, the executive and legislative branches work together to cut taxes for Americans. By doing this, it gives  people more discretionary income so that they can spend and stimulate the economy. i.e people and corporations have more money, which may make them more likely to buy things, which increases demand.Once the economy stabilizes, the government can gradually reintroduce the taxes and help keep the economy and the government going. †¢Increase Government Purchases Another way that the government can use fiscal policy to stabilize the economy during a recession is to increase government purchases. The government can use more money to buy goods and services from domestic providers. This increase in sales helps stimulate the businesses. This increases demand for labour, which can lower the unemployment rate.These businesses can then use this increase in income to buy more supplies and expand even further. Once this begins to happen, it can have a positive effect on the entire economy and stabilize the recession by providing more jobs and opportunities for unemployed entrepreneurs. †¢Expansionary vs.Contractionary – One of the arguments among economics on how to use fiscal policy centers around expansionary and contractionary strategies. An expansionary fiscal policy involves increasing government expenditures or lowering taxes so that the deficit increases. By comparison, a contractionary fiscal policy cuts back on government expenditures or increases taxes so that the government can have a financial surplus. Using an expansionary policy can improve the economy in the short-term, but eventually it could hurt the economy as the government’s debt becomes too large. Monetary Policy During an economic recession, unemployment rises while incomes, business investment and consumer spending fall. Monetary policy aims to shorten recessions by encouraging consumer spending and investment. Monetary policy actions can help shorten recessions or reduce their impacts, but economic conditions may limit their impact. In addition, it takes time for policy decisions to be felt throughout the economy at large. Government usually responds to an economic recession through stimulative fiscal policy, expansionary monetary policy or a combination of the two. Stimulative fiscal policy involves higher government spending in an attempt to stimulate the economy. Expansionary monetary policy consists of actions by central banks,  such as the U.S. Federal Reserve, RBI to expand the money supply to encourage more consumer spending and business lending. Expansionary monetary policy actions to battle a recession include the purchase of government bonds by central banks, reducing banks’ reserve requirements, and lowering short-term interest rates. Effects Of monetary policy- The purchase of government bonds by central banks injects more money into the economy. Lower reserve requirements give banks more money to lend because they are required to hold fewer reserves against deposits. Increased lending by banks stimulates business investment and expansion. A reduction in short-term interest rates also encourages more investment by reducing the cost of borrowing. Lowering short-term interest rates also reduces the rates on home mortgages, lowering mortgage payments for homeowners, giving them additional disposable income. Although expansionary monetary policy has the ability to reduce the length and severity of an economic recession, there is no guarantee it can do so. Lower interest rates, for example, may not stimulate consumer spending if consumers have little confidence in the economy. They are unlikely to increase their spending if they believe their jobs are at risk because of a sluggish economy. Businesses may be reluctant to invest in new facilities and equipment for expanded operations if the economy is in a recession. Finally, banks may be unwilling to increase their lending during a recession. Time Frame †¢Another concern about the ability of monetary policy to impact a recession is that the effects of policy decisions, such as a cut in interest rates, will not be immediately felt. It can take more than a year for the effects of lower interest rates to be felt. †¢During the 1991 and 2001 recessions, Federal Reserve policymakers repeatedly cut short-term interest rates to stimulate investment and consumer spending. It took time, however, for the effects to be felt. In 2001, for example, a series of Fed cuts reduced short-term interest rates to near zero. However, consumer uncertainty about the future, resulting from the 9/11 terrorist attacks, coupled with a  distrust of corporate accounting practices resulting from the collapse of Enron, blunted the effects of Fed efforts to expand the money supply. †¢In the end, the course of a nation’s recession is controlled by the actions of everybody living in the country. Anything influenced by so many people is beyond the control of any one person or group — it seems to have a mind of its own. But in the United States, time has proven that attitudes and economic factors shift, and every recession is a temporary recession. Eventually, things turn around and an upward spiral is reestablished. In the face of an economic collapse, the role of the government is invaluable. Governments have the power to avert an impending economic and financial disaster. 1. Encourage exports. The government should focus on the export business segment because it would infuse necessary foreign currencies into the country which would be used to pay debts, import goods and other necessities. 2. Provide Accessible Credit for Business. Local businesses should be encouraged by the government to compensate for unemployment thru extending credit to them. More businesses mean more jobs for the people. Or at least, source of income for the family. 3. Improve Tax Collection. Implement speedy and effective tax collection measures. Taxes can finance government expenditures such as provision of credit to businesses or budgets for social welfare. 4. Set Aside Large Amount Amount for Social Welfare. This will quell panic and riots and restore confidence in the people. Positive outlook will be developed in the process. This will also enable people to get back of their feet and start anew. 5. Control Expenditures in Other Fields. Slash budgets on unnecessary expenditures in other areas – military, legislative, executive, other branches. 6. Improve Tourism. Lure more tourists to the country. More tourists mean more money injected to the economy. Businesses will naturally sprout even small businesses in order to cater to the needs of these tourists. The main problem is the lack of funds as businesses closed and investors pull out their investments. The solution is to encourage the injection of money back to the country. Focus on the solution.

Comparing and Contrasting Beowulf and Odysseus Essay

The characters of Beowulf and Odysseus can be considered a contrast between ice and fire wherein one and is violent and uses its strength to consume and subdue while the other is cool and calculating preferring a patient yet affective approach to solving problems. When referring to the characteristics of fire and ice I am referring to Beowulf as fire and Odysseus as ice this is due to the fact that Beowulf uses his brute strength and force of will to combat his foes while Odysseus uses his cunning and wit to trick his foes into submissions. Analysis of Beowulf In the epic of Beowulf, he conquers and defeats 3 enemies, first is Grendel, then the mother of Grendel then finally a dragon. In all three instances Beowulf comes out on top not through using his mind rather by using his enormous strength. In the fight between him and Grendel he used his bare hands to kill the creature since no mortal weapon could supposedly kill it, when fighting the mother of Grendel he was able to escape death due to the armor he was wearing and was able to defeat the creature through the sheer luck of finding a massive sword nearby capable of beheading it. Towards the end of the epic when Beowulf fought the dragon he was able to defeat it only at the cost of his own life and this was due to the fact that his men abandoned him when they saw the fearsomeness of the dragon. From all these examples you can see that Beowulf is a character similar to that of Hercules in Greek mythology who is a slave to his emotions, uses his brute strength to solve problems instead of his brains and in the end succumbs to death by means of fire (Hercules died on a funeral pyre while Beowulf was mortally wounded by a dragon which is an embodiment of fire). We can conclude that in a sense Beowulf was a powerful hero who was famous for his strength but not for his brains, this particular characteristic might be due to the fact that since epic of Beowulf was Germanic in origin and the area at the time of the poems writing was a harsh wasteland the poem embodied elements that would have been relatable to people at the time who valued strength to endure the harsh the environment rather than intelligences. Analysis of Odysseus Odysseus is presented in both the story of Troy and that of the Odysseus as a person who possessed a modicum of great strength but was more famous for his wisdom and intellect. In the story of Troy he was the one who devised the means of entering the gated city by using a massive wooden horse that contained hidden troops that able to enter the city walls, open the gate and let the invasions force in. Throughout the Odyssey we see that Odyssey continuously uses hs mind to escape dire situations, from his escape from the Cyclops that tried to eat him and his men, to charming the witch Circe, to convincing the Phaeacians to lend him a boat to get back to Ithaca, throughout the story we see how Odysseus uses his mind to triumph and escape from most situations. In a sense we can say that Odysseus is the embodiment of a hero that the ancient Greeks admired since he possessed all the qualities that they valued namely an intelligent mind, great strength and above all empathy for his fellow man. Analysis of Beowulf and Odysseus All in all both characters were embodiments of the traits valued by the regions where their tales emerged as such you can say that both Beowulf and Odysseus were ideals that the local populace in their regions wanted to become or live up to. In the end both heroes have their own defining traits and it isn’t exactly a case of who is the better hero but rather who better embodied the spirit of the region they were created in. Blood Glucose Tests and Medication Definition: A blood glucose test measures the amount of a type of sugar (glucose) in the blood. Glucose comes fromcarbohydrate foods. Blood glucose levels that remain high over time can damage the eyes, kidneys, nerves and blood vessels. The importance of this test: This test is used to evaluate blood glucose levels. It may be used to diagnose or screen for diabetes and to monitor control in patients who have diabetes. Most dietary carbohydrate eventually ends up as glucose in the blood. Excess glucose is converted to glycogen for storage by the liver and skeletal muscles after meals. Glycogen is gradually broken down to glucose and released into the blood by the liver between meals. Excess glucose is converted to triglyceride for energy storage. The major hormone regulating glucose concentration in the body is insulin (although other hormones such as glucagon, epinephrine, and cortisol also affect it). Glucose levels are measured to diagnose diabetes or to monitor adequacy of diabetic control. Diabetes is a very common disease affects about 2% of the general population results from insulin deficiency or insensitivity by the body to the level of insulin present. People with type 1 diabetes require daily injections of insulin to control their disease. Injection of too much or too little insulin can be dangerous because there is a limited range of blood sugar levels in which the brain can function normally. Random blood sugar (RBS)Several random measurements taken throughout the day. Random testing is useful because glucose levels in healthy people do not vary widely throughout the day. Oral glucose tolerance test: A blood test done to diagnose diabetes mellitus. Its also done to diagnose hypoglycemia (low blood sugar) or a malabsorption syndrome in which sugar is not absorbed properly through the intestines into the bloodstream. The test was designed originally to determine the tolerance for the sugar glucose. Tolerance refers to the bodys ability to handle (tolerate) glucose. The test depends on a number of factors: The ability of the intestine to absorb glucose, The power of the liver to take up and store glucose, The capacity of the pancreas to produce insulin, The amount of active insulin it produces The sensitivity of the cells in the body to the action of insulin. Fasting overnight is important for this test and the patient is given 100 grams of glucose by mouth and then your blood glucose levels are monitored for 3 hours. Normally, the blood glucose should return to normal within 2 to 2 ½ hours. The outcome of the test may indicate: Normal glucose tolerance Abnormal glucose tolerance Depressed glucose tolerance: in which the blood glucose peaks sharply before declining slower then usual to normal levels as in: Diabetes mellitus Increased glucose tolerance: in which the blood glucose levels peak at lower than normal levels as in the: Malabsorption syndrome, Insulinoma (an insulin-producing tumor) Since the dose of glucose is taken by mouth, the test is sometimes called an oral glucose tolerance test. This test is commonly used to diagnose diabetes that occurs during pregnancy (gestational diabetes). Glycosuria Glycosuria refers to sugar in the urine. Less than 0.1% of glucose normally filtered by the glomeruli appears in the urine, and less than 130 mg should appear in the urine over a 24-hour period. Glucose is present in glomerular filtrate but is reabsorbed by the kidneys proximal tubule. If the blood glucose level exceeds the capacity of the tubules to reabsorb all the glucose present in the glomerular filtrate, the renal threshold is reached and glucose spills into the urine. Finding of glycosuria indicates hyperglycemic or lowered renal threshold for glucose. The renal threshold for glucose is 160 to 190mg/dl of blood; glucose does not appear in the urine until the blood glucose rises above this level. Glycosuria may be a normal finding, such as after eating a heavy meal or during times of emotional stress. Some individuals have a benign condition in which they have a lower than usual renal threshold for glucose (120 mg %), but have normal blood glucose levels. In pregnancy, the renal threshold for glucose may be lowered so that small amounts of glycosuria may be present. Patients on hyperalimentation may have glycosuria if the carbohydrate solution is being infused faster than the pancreas can produce insulin. The most common reason for glycosuria is diabetes mellitus. Urine glucose tests are used to screen for diabetes, to confirm a diagnosis of diabetes, or to monitor diabetic control. Blood sugar test results: Normal Results fasting plasma glucose test: 55-109 mg/dL oral glucose tolerance test at two hours: less than 140 mg/dL glycated hemoglobin: 3-6 percent fructosamine: 1.6-2.7 mmol/L for adults (5% lower for children) gestational diabetes screening test: less than 140 mg/dL urine glucose (random semi quantitative): negative Abnormal results: A fasting blood glucose level is 126 mg/dL (7.0 mmol/L) or higher. A 2-hour oral glucose tolerance test result is 200 mg/dL (11.1 mmol/L) or higher. Symptoms of diabetes are present and a random blood glucose test is 200 mg/dL (11.1 mmol/L) or higher. If your fasting blood glucose level measures in the range of 100 mg/dL (5.5 mmol/L) to 125 mg/dL (6.9 mmol/L), you are considered to havepre diabetes. Many forms of severe stress (for example, trauma, stroke, heart attack, and surgery) can temporarily increase glucose levels. Drugs that can increase glucose measurements include the following: Corticosteroids Diuretics Epinephrine Glucagon Epinephrine: It elevates the blood sugar level by increasing catabolism of glycogen to glucose in the liver, and also begins to break lipids in fat cells. Glucagon: Glucagon causes an increase in blood glucose, by stimulating gluconeogenesis and glycogenolysis and facilitating glucose release from hepatocytes. Low blood glucose stimulates the alpha cells of pancrease to release glugacon. Corticosteroids: These increase blood glucose by inducing glucose release from hepatocytes and inhibiting glucose uptake by cells by decreasing GLU-4. Corticosteroids stimulate gluconeogenesis and glucagon secretion (that increases blood glucose). Drugs that can decrease glucose measurements include the following: Alcohol Anabolic steroids Clofibrate MAOS inhibitor Alcohol: After the ingestion of the food, the body turns the food to sugar or glucose. The pancreas produces insulin to help move the glucose into the bodys cells for energy use. When insulin is working effectively blood sugar levels are stable. Although the process of getting into this occupation is quite simple, there is a lot of work to be done in this growing field. Almost anyone can get a degree in nursing, but it takes a special kind of person to be a hospice nurse. This is because hospice nurses deal with the emotions the come with caring for terminally ill patients every day. How to Become an Effective Hospice Nurse Hospice nursing is an occupation that many people dont know about, although it deserves as much credit and awareness as any other nursing position. Many hospice nurses go to great lengths and make huge sacrifices in order to get this position, and the process to do so is very intricate and can be confusing. Nursing is a profession that takes a special kind of person, especially hospice nursing, because their job is to assist a patient and the patients family through a death. In todays healthcare situation, there is a rapidly increasing shortage of nurses. This is due to the fact that most registered nurses are retiring all at once, and there is no one there to fill their shoes. It is not uncommon to go to the doctors office and not give a second thought to the nurse seen before and after the doctor. This is part of what makes hospice nurses different. They form a close relationship with not only the patient, but the family as well. As with any medical profession, it can be difficult to understand how a person went about obtaining their career. There are many routes available, along with characteristics that make up a good hospice nurse. Essential Question How does a person become an effective hospice nurse? Supporting Questions What schooling is required, and what kind of classes should be taken? What kind of certification is needed, and how is it obtained? What is the job outlook, and why is it like this? How will having certain characteristics and specific skills benefit? What schooling is required, and what kind of classes should be taken? To be accepted into nursing school, a person has to have a high school diploma. Taking classes in math, biology, and chemistry while in high school help build a strong application, as well as being able to speak a foreign language. Someone looking into being a registered nurse can choose one of three options when it comes to finding a program. The first option is to get a four-year bachelors of science degree in nursing. This is offered by more than 700 colleges. The second option is to get either a two-year or a three-year associate degree in nursing, and this is possible at 850 community colleges. The final alternative would be to go through one of the 70 diploma programs available at some hospitals, which takes about three years (Education and Training, 2009, p.3). Regardless of which path is chosen, all nursing education will involve supervised clinical and classroom instruction. Anatomy, physiology, microbiology, chemistry, nutrition, psychology, and behavioral sciences are some of the classes that are commonly taken by nursing students (Paying Your Dues, 2010, p.1). It is important to take math, biology, and chemistry in high school because even if they cant be taken as college credit, they are still crucial preparatory classes. They will serve as the basic foundation for starting out in nursing education. Being able to speak a foreign language is helpful as well, because people from all backgrounds and cultures need medical help and deserve the right to speak in their own language. This is especially true in the hospice area of nursing because most patients are being taken care of in their own homes, and need the comfort of not having to be stressed about what languages their nurse can or cannot speak. One of a nurses main duties is to serve as a central communicator between the doctor and patient. The best option to choose when picking a nursing program is getting a four-year bachelors of science degree (BSN). It used to be that a nurse with an associate degree could get a job at almost any hospital. However, times are changing and it is now p referred that nurses have a four-year BSN. It is important for all of the programs to have experiences in clinics in all different settings because it provides students with a chance to learn about multiple areas of healthcare. For instance, a person could be planning to have a career in geriatrics, but after spending hours at a nursing home for a clinical, they might completely change their mind. What kind of certification is needed, and how is it obtained? In the United States, all hospice nurses must be registered nurses in addition to being certified by their states health department as a hospice worker. In order to become a registered nurse, a person must take and pass a national examination after graduating from a school of nursing (Education and Training, 2009, p.3). This examination is called the National Council Licensure Examination, or NCLEX-RN (Licensing and Certification, 2010, p.7). It is mandatory for all registered nurses to renew their license periodically, and some states require continuing education. This test is administered electronically, and is multiple-choice. Students usually begin studying for it in their final year of nursing school. Results are sent by mail and arrive five to eight weeks after the testing date. Being certified by the state health department as a hospice worker is important because it ensures that only nurses trained for this field will be entering palliative situations. If this requirement wasnt in place, any registered nurse could go into a home and perform hospice work. It is because of this rule that patients and families can feel confident that they will receive the best care. The National council Licensure Examination helps weed out the students who arent quite ready to start their career in nursing, and need a little more time to study. The students who dont pass can continue to study and try again the next time the test is administered. What is the job outlook, and why is it like this? Registered nurses have more than 2.5 million jobs in the United States. This is expected to hold through and only increase until at least the year 2016. The growth of healthcare in general and the needs of a quickly growing and aging population will raise the demand for registered nurses. The Bureau of Labor Statistics says that more new jobs are to be created for registered nurses than for any other occupation (Employment Outlook, 2010, p.4). The number of elderly is increasing rapidly, which creates many new jobs for hospice nurses. Another reason is because patients are becoming more and more okay with the idea of in-home care, and are actually starting to prefer it. Technology is supporting this by allowing nurses to deliver complex treatments away from hospitals. As interest in the field of hospice nursing grows, so do the opportunities. Right now there are two programs in the United States that provide masters degrees that focus on hospice care. One is at New York University, and the other is at Ursuline College, which is in Pepper Pike, Ohio. However, once this career takes off, there will hopefully be more masters degree programs available that specialize in hospice nursing. How will having certain characteristics and specific skills benefit? Hospice nursing includes 24-hour availability, pain management, and family support combined with compassionate listening and counseling skills (Hospice/palliative care nurses, p. 1) . According to Theresa Valiga: Nursing is a profession that challenges a person intellectually, physically, and emotionally. But it is also a profession that is incredibly rewarding. Nurses are with people during every major life event from birth to death. They have the privilege of being quite intimate with strangers. They are well-respected and they make a real difference in the lives of people and communities. Who could ask for more from a career? (p. 1) Like she said, a hospice nurse needs to not only be able to handle the physical stress of the everyday work, but also the emotional stress. One of the main goals of hospice nursing is to find out the patients last wishes and make sure they are carried through and communicated to the family (A. Fredericks, personal interview, December 9, 2010). On average, hospice patients die within a month of enrolling in a hospice program. Because of this fact, hospice nurses need to have skills to deal with spiritual and culturally sensitive patients. They also need to be able to lift patients or help them bath, sit up, get out of bed, or walk. Sometimes the patients are moody, confused, and uncooperative. Communication is a huge key in hospice nursing, because it is the nurses job to observe the patients condition and report it to the doctors, as well as the patients emotional needs and wishes to the family. How does a person become an effective hospice nurse? The first step to becoming an effective hospice nurse is getting a bachelors of science degree from a four-year nursing program. After that, the National Counsel Licensure Examination needs to be passed, as well as registering with a state health department as a hospice worker. A masters degree may be obtained specializing in palliative care. The psychological aspects are different for each person it depends how they react to the thought of a person dying, some people may feel that it’s their own fault or that they have done something wrong taking the blame for it or someone may feel as if their being useless and feel like there’s things that can be done to help but in actual fact there isn’t. In the person’s last days if known others may like to spend a lot of time with the person but others may not want to see them like that and just want to have the better memories as people can’t handle seeing the person looking very frail as this cannot be very nice, but it all depends on how each person can handle the situation and it is also hard as you don’t know whether to expect the worst each time you go into their room as they could have passed away. It can cause a lot of stress within the home setting and people will be very sad but we have to do our best to try and keep them as happy and as calm as they can be. Common signs of approaching death, you can never know when someone is going to die but there is always a sign such as they May- Lose their appetite the person may begin to resist or refuse to eat meals or any drink fluids. Become very fatigue or sleepy- The person may begin to sleep the majority of the day and night as metabolism slows and the decline in food and water contribute to dehydration. Physical weakness- As the person is declining food and sleeping a lot they will find that they have no energy. Confusion or disorientation- Organs begin to fail including the brain they may start to forget where they are or what they are doing. Change in breathing- Breath intakes and exhales can become very irregular. Social withdrawal- As the body starts to shut down they may gradually lose interest in being around those that are close to them. Swelling in feet and ankles- Coolness in tips of fingers and toes. Good end of life care helps patients with life-limiting conditions to live as well as possible until they die, and to die with dignity. End of life care encompasses palliative care, which focuses on managing pain and other distressing symptoms, providing psychological, social and spiritual support to the patients, and supporting those that are close to the patient. All patients should be treated with dignity and respect throughout the course of their care. Patients who are dying must receive the same standard of care as  all other patients. Their privacy, dignity and wishes must be respected at all times, by listening and achieving their wishes can make them feel more relaxed and less distressed. Making sure your patient has good pain relief, making sure they are always comfortable, having other people around them to keep them occupied or even little things like listening to the radio or watching television if they enjoy doing this. Making sure your patient has everything and every one they need around them, some people may not want to be like this some people may want to go alone and this needs to be respected even if their family members or loved ones want to be with them you have to respect the fact they want to be alone as long as they are comfortable. There are rules governing when life-prolonging treatment can be withheld or withdrawn when the patient’s consent is not available, the relatives and friends of the person receiving the treatment and the healthcare professionals in charge of the person’s care, should decide whether treatment should be continued or not. They should consider whether the treatment is in the best interests of the client in terms of quality of life and how long they will live and if the treatment is helping to improve their condition. If the patient still has the capacity to decide whether or not they want to carry on or stop with the treatment it is their own choice as they still have the capacity to their rights. Also it needs to be remembered that the client should always be center of the care. Some of the signs that death has occurred are they- Have stopped breathing No heartbeat Loss of bowel and bladder control No response Eyelids slightly open Eyes fixed on a certain spot Mouth slightly open The body cools When a client does become to near the end of life or even ill from the start you have to work with other professionals at first you may start with the GP to diagnose the patient and they would then refer them to the hospital if required if not they would give you prescriptions for the right medication  to treat them. You would also work with district nurses if required if a client had a bed sore. You would work with the pharmacy to make sure you receive all the medication and on time and to order when needed. A patient should be monitored regularly; everything that the patient does should be recorded and documented correctly making sure they are legible, accurate, relevant and clear. Things that may get monitored- Food intake charts Fluid intake Charts Continence checks Positioning Chart Checking Temperature Body maps to mark if they have any bed sores If they were in pain or how much pain. Some people may not be able to tell you how much or if they are in pain so in some care homes they use pain rating scales, Faces tool, Disability distress assessment tool (DisDat), Body charts/ Maps or its even suggested if the patient is able to keep a pain diary. Everyone approaching the end of life should have their needs assessed and their wishes and any preferences discussed, any actions reflecting their choices they make about the care they would like to receive should all be recorded in their own personal care plan especially as some people in advance want to refuse treatment, the care plan should be regularly reviewed. Once a client enters into the dying phase the care providers need to adapt their care and support according to the care plan, also the client may have a preferred place of death which they had discussed in their care plan this should be reviewed to make sure the care providers are aware. Advanced care planning should be completed as it gives people the opportunity to make their wishes and preferences known should they deteriorate and lose the mental capacity to make informed choices. When a client’s needs change we make sure all staff and any other professionals are aware and know fully of all the changes and any requirements or adjustments that need to be made such as ways of working, making sure all changes are documented in their personal care plan, in staff communication books, also making sure that all risk assessments are updated, reviewed and changed as needed. Making sure the client has everything that they need to meet their needs to the changes. This also helps everyone to  follow the same way of working to support the client when any changes do happen. Some family members/carers may wish to assist with the personal care in acknowledgement of individual wishes, religious or cultural requirements. If it is down to the staff to carry out this task then the Staff should be Prepared before they do this as there are changes to the body after death and they should also be aware of manual handling and infection control issues. Some people may not want their family or staff to do so, once the patient has died you should refer to their plan of care to see and follow their wishes if any after death. In private nursing homes the personal care after death is the responsibility of a registered nurse, although this and the packing of the property may be delegated to a trained healthcare assistant. The registered nurse is responsible for correctly identifying the deceased person and communicating accurately with the mortuary or funeral director (in line with local policy). In care homes without a registered nurse, the home manager is responsible for ensuring that professional carers are trained appropriately and to ensure that they are competent in their role. The death should be documented straight away the time the date and the people that are present, this also has to be reported to the care quality commission within 24 hours. All staff knows and are fully aware that when a client has passed that all procedures in place must be followed when handling the decease and moving their belongings, gloves and aprons must be worn, all of their personal belongings such as bedding, bed cloths and clothing should be disposed of in the correct way. Everyone is different to how they react to somebody passing, there are different types of ways such as- Emotional- Sadness, anger, blame, depression and loneliness. Physical- Weakness in muscles, hollowness in stomach, tightness in throat and shortness of breath. Cognitive- A person may experience mental difficulties, such as poor concentration, forgetfulness and day dreaming. Behavioural- Some people may react by crying, loss of interest and withdrawal. Spiritual- They may start to question their spiritual beliefs but others may find solace. When telling family or friends that their loved one has passed you have to be prepared for different types of reactions as above, people should always be allowed to spend time by the deceased and their privacy should always be respected. Offering a cup of tea and going into a quiet space where they will not be disturbed so that they can talk about what happens next, sometimes people may find it hard to absorb any information so the care staff should write up a simple and clear guide for them. When talking or telling close family or friends you should always use good listening skills show empathy, always make yourself available and give people time to express  themselves and talk, offer any help to register death or organise a funeral or even just offer any advice. In a care setting the rest of the house should be told what has happened and this should not be hidden from them as they all have the right to know, they should all be told in a tactful and sensitive way. Offer support and let them know that staff will always be available if they feel they need to talk.
